Description:
  • Plans, provides, and evaluates patient-centered nursing care provided by effective utilization of staff members to meet hospice patient needs.
  • Develops and coordinates hospice programs to ensure the delivery of quality patient care.
  • Ensures individualized, appropriate hospice plan of care through effective utilization of the interdisciplinary team.
  • Ensures appropriateness of patient admissions, discharges, and level of hospice care through consultation with interdisciplinary groups.
  • Participates in the patient/family assessment process and care plan development.
  • Coordinates physician instructions with nursing staff and internal and external hospital departments.
  • Requires RN license.
